BLACKBURN HALL CARD MEMBERSHIP
Attention 6, 7th and 8th Graders! Get your Blackburn Hall Card membership. All participants must get a Blackburn Hall Card prior to their first dance. Dances take place monthly through the school year and you must have a membership card to attend.
The fee for membership is $25 payable with your membership application each year. All dances have an admission fee of $5. Parents who chaperone at least two teen activities during the school year of the student’s membership will receive a $20 refund.
STEPS TO GET A MEMBERSHIP CARD:
1. Download and fill out the dance card membership application listed below. Signatures are needed by both the child and adult.
2. Bring this form and payment of $25 to the Walpole Recreation Department office. If you choose to pay online with a credit/debit card, please bring the receipt with you. You will still need to fill out the application form and bring it also. A picture will be taken by the Recreation Department of your child for the dance card. You can then pick up your dance card at the first dance.
